We are committed to making sure people living with dementia and their families receive the support they need, where they need it. Our services are designed to be local, specialist, and person-centred, recognising that no two communities – or individuals – are the same.

This is where our work began and is still the area where we offer the widest range of services including peer support and community groups, one-to-one Dementia Coordinator support, Singing Back the Memories and access to The Beacon Day Support Centre.
In Medway, we provide essential Dementia Support and Dementia Wellbeing services that help people access support early, navigate the diagnosis process and connect with local opportunities to live well with dementia.
Our Dementia Coordinators offer specialist one-to-one support across Swale, working with families and professionals to make sure people get the right help at the right time. We also provide some Dementia Wellbeing services, working in partnership with Age UK Sheppey and Age UK Kent Rivers to expand the support network for people in this area.
Covering the districts of Sevenoaks, Tonbridge & Malling, Tunbridge Wells, and Maidstone, our team of Dementia Coordinators delivers practical and emotional support to people at all stages of the dementia journey.
While we are passionate about reaching as many people as possible, the services we provide depend on local commissioning and other funding arrangements. Currently, we are contracted by the NHS to deliver Dementia Coordinator services in the areas listed above.
This is because we started in Dartford, Gravesham and Swanley, our services there are the most developed, with a wider variety of group activities, support options, and established community partnerships. In other areas – such as Medway, Swale and West Kent – we are building strong foundations through our Dementia Coordinator teams and working closely with local stakeholders to grow the range of support available.
We’re always exploring new opportunities to expand our work across Kent and Medway, and we welcome conversations with commissioners, community leaders, and partner organisations to help make that possible.
